Treatment planning:

Once your treatment strategy was agreed with the Dentist, we'll provide printed information on your strategy. Whether this strategy shifts due to clinical or radiographic findings, we discuss this with you and will notify you. Treatment strategies are not invalid for net 90 days from the date. Before the appointments for genuine treatments are made if there are any points in your treatment strategy that you wish to query, please do not hesitate to inquire us.

Approval forms:

Particular treatments necessitate completion of a written consent form. This is to be able to describe the treatment, aftercare and any risk to you personally completely, before any of these treatments are carried out.

Fees:

A credit account system does not manage and we require fees to be settled at the appointment.

Pearl Dental Clinic does not accept any payments.

Fees for treatment where intravenous sedation is included, must be paid before the appointment to prevent fiscal transactions, or signatures being demanded, whilst a customer remains under the influence of the sedative.

Deposits are deductible off the price of treatment.

Dream Smile Dental Clinic reserves the right to make a fee for any debt passed to some debt collection agency.

All the fees applied for any form of Dental treatment at the clinic are based on; the materials used, lab costs incurred ( in case that it's appropriate), and the clinical time spent. The fees are yet, demonstrated as treatments prescribed on the treatment strategy for your information.

Late cancellation or missed appointments:
Dream Smile Dental Clinic reserves the right to charge up to?100 per hour, in the function of an appointment or a missed appointment. For appointments more than 1 hour, we require at least 72 hours.

Late for appointments:

Guarantee:

Ceramic Veneers, at Dream Smile Dental Clinic our Lumineers, Implant crowns and Ceramic crowns are guaranteed for a period of 5 years following placement, provided that the following conditions are satisfied:

The patient has completely paid for the treatment and does not owe the practice any cash for the treatment received.
The Veneers, Lumineers, Crowns & Implant crowns have not been damaged as a result of an accident, trauma or excessive grinding.
The patient has followed all post treatment care recommendations made by our dentists.
The patient has attended and routine assessment every 6 months.
Private Details:

It's crucial that you give a full medical history and details of any medication you take. Should these change at all, it really is essential that you tell your Dentist. It truly is the patient's obligation to notify the clinic of any changes in their medical history private details and/or.

Use of Pictures and X-rays:

Dream Smile Dental Clinic may use pictures and x rays of teeth and your smile just, for educational and advertising purposes on the Dream Smile Dental Clinic site, and on educational and promotional literature. Criticism's policy:

At Dream Smile Dental Clinic we consistently take complaints about any aspects of our services very seriously, to be able to ensure that each patient has just the best experience at all times.



Every criticism will receive immediate written/email acknowledgment, and the Management will strive to resolve the grievance within a fast, fair time period (normally about 2 weeks). No tolerance/Mistreatment policy

At Dream Smile Dental Clinic we manage a zero tolerance policy to mistreat to our Dentists and staff, loud/disorderly/drunken behaviour, persistent overlooking and late cancellation of appointments (after multiple warnings). In these scenarios, Dream Smile Dental Clinic reserves the right to refuse treatment and entry.

Data Protection Act:

We store all patient personal details on a protected computer system relative to the Data Protection Act. All clinical notes, digital radio-graphs pictures etc that are digital, remain the property of Dream Smile Dental Clinic. Copies of radiographs, notes and photographs can be made accessible and Dream Smile Dental Clinic reserves the right to charge an administration fee for these.
